Two passport sized color photos affixed with the application form.
Self attested photo copies of marksheet and certificate as educational
qualification proof.
Self attested photo copies of computer certificate.
Original copy of residential certificate issued by the pradhan of the
concerned panchayat.
Self attested photo copy of Voter card (Epic card).
Self attested photo copy of madhyamik admit card.
